If it's just a blood test it should be fine. Each country has at least one decent private hospital, Kazakhstan has the best ones I think. For anything serious people fly to Dubai or Istanbul or India.

Yes, both Baku and Aqtau are more business travel than tourist destinations so they are used to getting your business anyway. Sounds closed. I don't have details. It was a nice place back in the day when options were very limited for budget travelers. These days you do have a bunch of options (although it pays to book in advance in high season).

We don't work with any rental companies in Uzbekistan anymore cos we couldn't honestly recommend anyone decent...I agree it is a problem to find someone reliable. But you certainly don't need a 4x4 for Uzbekistan, it's mostly flat and the roads are decent. So if it's just you, get the cheapest car y...

Seeing how the most common vehicle in rural areas is still a beat-up Lada from Soviet times, it is certainly possible to do it without a 4x4. I've driven all over Central Asia in a 1998 Honda CR-V. Repairs were needed but we got everywhere on the local fuel.

For question 1. You can take a ferry with the car across the Caspian Sea from Baku to either Turkmenistan or Kazakhstan.

There are always these little agencies popping up. It's usually former guides from Owadan who decide to take some clients with them and start on their own. It usually ends quickly as these people do not have a license.
