Depending on the mood of the border guard, Iran may refuse entry to this car, since alcohol is prohibited and they could consider it to be encouraging to violate the prohibition. On Kyrgyz Railways' website there is still the information from last year, which states that the train runs the last time on 31th August (2018). However, I took in on September 8th, which means it ran longer than announced. Let's wait and see...

Neither "Kazakhstan" nor "Dushanbe" are in Uzbekistan, and also, Dushanbe is not situated in Kazakhstan. Apart from that, normal medication is no longer a problem in any of these countries.

Tajik Railways have a fancy new website: https://www.railway.tj/ru/
It's loading very slowly, and I was really impressed that they say online booking was possible now (although I haven't tried it out)!
Only Russian/Tajik though.

Totally impossible. Other direction is possible but you might run into trouble when going back to Georgia on a later occasion.

It appears that currently Afrosiyob tickets are put online for sale only 14 days in advance. No idea why. Your alternatives are booking a slower, cheaper train (which are still perfectly comfortable IMO) or waiting.
